Overview of Position:

Provides professional social work services through the assessment and resolution of problems and issues that impact overall health and well-being, including discharge planning.

ESSENTIAL DUTIES

1. Utilizes supportive counseling techniques, provides crisis interventions, and facilitates therapeutic, support, and education groups as needed to assist patients in developing coping strategies.

2. Completes psycho-social assessments utilizing a systems approach.

3. Facilitates completion of Advance Directive forms, effectively completes Pre-admission Screening form for Mental Illness and Mental Retardation, and verifies appropriateness of patient need for guardianship.

4. Demonstrates ability to practice independently as outlined by Ohio Counselor/Social Work Board and when delegated by manager, provides supervision to LSW, LPC, and students.

5. Provides appropriate interventions to patients and families during times of emotional, psychosocial or financial stress as related to their medical conditions.

6. Acts as patient advocate, mediator, facilitator, supports patient’s right to self-determination, and demonstrates compassion, warmth, and sensitivity to patients.

7. Involves patient/family/significant other in any decision making related to patient’s plan of care.

8. Intervenes effectively with the more involved, complex family and patient interventions.

9. Assists with orientation of new staff and/or students

10. Monitors length of stay (LOS) and identifies and participates in the development and implementation of strategies to reduce unnecessary resource consumption on assigned patient populations.

11. Collaborates with members of the medical staff and the interdisciplinary team to ensure efficient delivery of care.

12. Keeps abreast of current changes in law which affect social work practice.

13. Works closely with physicians, nursing staff, allied health professionals and RN Case Manager to promote positive patient outcomes.

14. If cross-trained and credential allows, will function as Behavioral Health Therapist and/or Chemical Dependency Counselor.

15. Follows the ethical codes as established by the State of Ohio Counselor and Social Work Board.

QUALIFICATIONS

1. Master of Social Work/Counseling with current license in the state of Ohio. Hospice/Home Care requires a Masters of Social Work.

2. Highly developed verbal and written interpersonal skills required.

3. Extensive knowledge in human behavior and personality development required.

4. Ability to use a holistic approach to patient care.

5. Valid driver’s license, and proof of auto liability insurance.

6. Demonstrates knowledge and utilization of resource networks.

7. Knowledge of all Medicaid waiver programs and makes referrals as appropriate

8. Knowledge of physical and psychological indicators of abuse and neglect and follows protocols in accordance with state law and hospital policy.

3. Promotes Patient and Employee Safety

  • Demonstrates safe Patient Handling (i.e. transfers, transport, care administration, nutrition, medication, etc.)
  • Demonstrates safe Materials Handling (i.e. appropriate use and disposal of chemicals, infectious wastes, etc.)
  • Demonstrates appropriate knowledge of Infectious Disease precautions and use of proper protective equipment
  • Demonstrates Slips/Trips and Falls Awareness.
  • Actively contributes to maintaining a safe, clean and quiet environment.

WORKING CONDITIONS/PHYSICAL REQUIREMENTS

1. Hear and interpret responses to evaluate progress.

2. Able to stand for one hour, sit for two hours. May need to bend twist, kneel, stoop and reach.

3. Able to deal with aggressive patients/participants both verbally and physically.

4. Must be able to lift up to 50 pounds.

5. May be required to periodically rotate shifts and regular days off.
