Social Worker – Adult Hospital & Community Job Details:

Offering a fantastic opportunity for UK-registered Social Workers to experience a typically Australian lifestyle without the ever-increasing cost of major Australian capital cities – to join the Hospital-based Social Work team in the stylish city of Hamilton, at the centre of the famous Western District in the state of Victoria.
Hamilton has a population of 10,000 and is located 291 km (180 miles) west of Melbourne via the regional cities of Ballarat or Geelong.
The salary range is up to $99,700 Australian dollars (approx. £53,500 £GBP) based on a years experience scale.As one of two Social Workers you will provide support for patients being discharged from Hospital with their transition back into their homes and community.
Ensuring the smooth transition as an outpatient may include monitoring recovery, supporting organising, and attending specialist appointments, provide supportive counselling, debrief and education relating to their hospital stay and advocacy for patients needs with recovery based organisations.Responsibilities and duties include:
To provide a confidential and effective social work/counselling service to inpatients, outpatients, and the community (individuals, small groups, and families), within a “social model of health” framework. This would require strong counselling and assessment skills to develop appropriate treatment goals, strong advocacy, and coordination skills to develop these plans and recognising when to work in collaboration with services such as District Nursing, rehabilitation / community services.
Duties will involve working with hospital consumers being discharged and develop their recovery plans with them. Active outreach monitoring in a person’s home environment may be required.
Alternatively, community members may seek support with accessing Allied Health services to progress applications for access support with National Disabilities Insurance Services (NDIS), Aged Care Pensions etc that may require reports from Allied Health Professionals.A hospital was first built in 1864 on the site of the current state-of-the-art facility serving the wider Western District and includes some smaller outlying rural ‘cottage’ hospitals with a patient catchment of over 16,000.
Inpatient facilities include; Theatre complex with on-site CSSD, Level 3 ICU, A&E, Chemotherapy Oncology Centre, Dialysis Unit, General Paediatric Ward, Maternity and Neonatal Unit, Medical Ward and Surgical Ward.
The busy Pharmacy, Imaging and Therapies departments employ a wide variety of Allied Health Professionals, including Occupational Therapists, Physiotherapists and Podiatrits.
